or provided as proof of child's identity

Copy of Medical/Health Insurance coverage must be
provided

Complete and return with Registration Fee to Enroll for the Fall 2012 Class.

If your child requires a prescription medication for any
reason during the PreSchool day, you are required to
complete the medication consent form. A form is
required for each medication.

Tuition is due between the 15th and 30th of the previous month:
